You're absolutely right of course it isn't in character for Dean to say that kind of stuff, there's always got to be something that makes him have to get beyond his usual limitations. And as has happened so many times it comes down to his "prime directive", he figured out that to take care of Sammy, he needs to actually say the words out loud for once.
